Evolve Care Group brings together decades of experience within the Residential and Nursing Home sector. The group specialises in supporting those living with dementia. It is one of the largest providers of Dementia Care within the South West. All homes within the Evolve Care Group are uniquely independent of each other however they do have the one unified ethos which is they all strive to be a true continuation of home; a place where everyone feels safe and never alone.

The group has developed its own research based bespoke model of care – known as the Evolve Household Model.
